Passed away Monday, April 16, 2012 at the age of 54 years. Visitation at Vorhis & Ryan Funeral Home, 11365 Springfield Pike, Springdale, OH on Monday, April 23, 2012 from 6:00 PM until time of funeral service at 8:00 PM.
